My first pick for lesson book (for this age and level), is Bastien Piano Library (traditional) Primer Piano Lessons, Technic Lessons, and Theory Lessons (three orange books), but they are out of print and hard to come by. This is Bastien's 'modern' replacement for the Piano Library series.
It's full color and has great cartoons, but the songs don't sound like songs (not even to 5 year olds). They are repetitive and mundane. The older series does a better job of including familiar folk ditties and tunes into the primer level books so that the student feels like he or she is learning something "cool." In addition, the combination Theory and Technic and division of primer level into two separate primer levels A and B, lengthens the time it takes to learn the material. They do a good job teaching the required theory and technic, in a basic way, but the theory work is mostly dry (not fun) and students don't get enough note spelling work. This sets the stage for future problems if you don't know what you're dealing with.
A mediocre book, all in all- they just don't make things the way they used to! I'll keep ordering my "old" lesson books and waiting 9 weeks for delivery...
